Difficult to tell but it might be a distant star forming region.
It's NGC7354, a planetary nebula. More info in the comments below and discussion "The Cosmic Peanut".
The tiny red dot might be - it's a tough call as the resolution is not so good on this image.
Red dots are either small star forming clouds or distant galaxies. Go with your instinct! Remember the MWP is currently in between datasets.
